Instructions for Using On-line Forms

All of the Environmental Sciences and Resources' forms listed below are on-line as Adobe Acrobat PDF files. To read and print these files, you must have Acrobat Reader 3.0 (or newer). This is a free program and may be downloaded by clicking on the Acrobat Reader icon to your left.

Once you have a copy of Acrobat Reader, you can open the downloaded forms, fill them out on your computer, and print them, or, if you prefer, you can print them and fill them out on a typewriter.

There are two ways to print out the ESR Office forms:

1. Using the Adobe Acrobat Reader plug-in with your web browser:

If you have installed Acrobat Reader 3.0 with the PDFViewer plug-in, you can view the forms and print them directly in Netscape. Just click on the link to the appropriate form and wait for it to open in your browser window. Then choose "Page Setup..." under the "File" menu to prepare it for your printer and choose "Print..." to print out your own copy.

2. Using Adobe Acrobat Reader as a stand-alone application:

If you are using an older copy of Acrobat Reader, or if you haven't installed the PDFViewer plug-in, you can still view and print the Graduate Office forms. Just click on the appropriate link and save the file to a convenient place on your computer. Then open the file using Adobe Acrobat and print it from within that application.

To fill the forms out on your computer:

Using Acrobat Reader, open the form and click your mouse on the appropriate region of the form and fill in the necessary information. In each form, there are imbedded notes which you should read. They are indicated by a yellow "note" icon. Double click on this icon and the note will appear. Please pay careful attention to these notes. Once you have completed the form, print it off and obtain the appropriate signatures.

Completed and signed forms may be turned in to the ESR Office, 218 Science Building II. Copies of all of these forms can also be obtained directly from the ESR Office. These forms may not be submitted electronically.

On-line Forms

Follow the admission offfices' guidelines for application. The corresponding ESR forms must be signed and turned in to the Environmental Sciences and Resources office in Room 218 Science Building II or mailed to:
